Election Forms

The following are forms which are to be used in an unit election.

• Checklists
• Checklist A - This checklist should be used by the election team before the election is conducted to make sure that the preparations are complete.
• Checklist B - This checklist should be used by the election team during the election to make sure that the elections are complete and accurate.
• Checklist C - This checklist should be used by the election team after the election to make sure that the election is reported properly.
• Unit Election Forms
• Election Announcement - An announcement sent to unit leaders describing the OA and the election process.
• Ballots - Sample ballots to fill out and use when carrying out the election.
• Election Reporting Form - The form to use when reporting the election results to the Lodge.
• Adult Member Forms
• Adult Member Qualifications - This flowchart shows the requirements for an adult member to join the OA. If the adult does not pass this chart then they will not be admitted.
• Adult Member Recommendation - This form should be used when recommending adult leaders for membership into the OA.
